Department of Health and Human Services Administration on Aging Back to Previous Evidence-Based Disease Prevention Grants Program The evidence-based disease prevention grants program is a public-private partnership of federal agencies and private foundations coordinating their efforts to support evidence-based prevention programs at the state and community level. Evidence-based prevention programs are interventions based on the application of principles of scientific reasoning, behavior change theory, and program planning that are proven effective in reducing the risk of disease, disability, and injury among the elderly. State and local programs that implement evidence-based practices will be better able to improve the health status of this vulnerable population. Awards These awards support the delivery of evidence-based programs for service providers of older adults, such as senior centers, nutrition programs, senior housing projects and faith-based organizations.
